Please make sure your items are described correctly on the BOL to avoid these additional charges. Proof of these charges will be provided at the time of the additional charge and will be included with the credit card receipt. I authorize SMITH-CARGO/SMITH TRANSPORTATION SERVICES INC. to charge the agreed amount listed above to the noted credit card provided herein for prepaid shipping services.
